Waiver

Please find our online waiver form below. You must be signed in to your Google Account to access and see the form below. After signing in to your Google Account, please refresh this page and fill out/submit the form below. If you do not own a Google Account, please request that we email you a waiver to sign and send back to us. You can do this by submitting a request from our Contact Us page.

Please note: Your waiver must be active during the school year in which you are participating in bike rides and PN2K related activities. Each school year runs from August to July. For example, if you are participating in a bike ride on May 5, 2018, you must have signed a waiver between Aug 1, 2017 and Jul 31, 2018. NO EXCEPTIONS.
